This serves as an agreement between myself (the customer) and Georgia Spa Company to perform selected EZ Valet Care Services on my sauna as indicated by the selected plan from the date this form has been filled out and submitted. Chosen plan and/or service agreements will automatically renew unless I notify Georgia Spa Company in writing that I wish to change or cancel my service option by the 25th of the month preceding the next month’s scheduled service. I understand my selected EZ package or service does not include any sauna repair, maintenance or warranty services of any sauna parts, controls or other service that would normally be covered by the manufacturer’s warranty or performed by a certified sauna technician. The EZ Valet coordinator will schedule the first appointment, any subsequent appointments, and note any special requests or service notes. *Depending on severity of stains/damage to wood, Georgia Spa might not be able to remove stains, but would recommend replacement.
